Concerns Process
Any complaints or grievances from the client regarding the performance or actions of a therapist shall follow this procedure:
1. A person having a complaint or grievance regarding the Therapist must discuss the matter with the Therapist in person before pursuing it in any other manner.

2. If, after discussion, the client decides to pursue a complaint, he/she must then submit a written letter detailing the complaint or grievance to the Therapist. The Therapist will provide a written response to the grievance within 30 days to the client. Both letters will be forwarded to Christy Graham, the president of Acorn Counseling Education Services. jones@nsighttherapyservices.com

3. If appropriate, the Therapist will then meet with the client and their attorneys (if any) to discuss the matter.

Records Requests
For clients requiring copies of their records, please submit a written request to our office. Please specify the type of information you are requesting — billing, notes, full file — and your relationship to the client. If you are not the client or guardian, you must provide an exchange of information form signed by the client or guardian. A fee for records can be found in your intake documents or requested at the same time as the records. No fee for billing information is assessed. If notarization is needed, please note there is a separate fee for this service.
